We have exciting opportunities for qualified Social Workers to join our adult social care teams based in Bath and North East Somerset. Our adult social care teams cover a variety of services including: Learning Disability Team (Transitions) The LD team undertake first adult care assessments and reviews with vulnerable young adults with learning disabilities, and their carers, and to work with service users to create care and support plans which meet their needs. This is a specific role working with young people (16-25) with the appearance of social care needs transitioning into adult care. You will be based out of Radstock but will work across the whole of Bath and North East Somerset region. Autism Team The Autism team are responsible for assessing and supporting adults who have been diagnosed with an Autistic Spectrum Condition (ASC), in line with the Care Act 2014. You will be based out of the Civic Centre in Keynsham but will work across the whole of Bath and North East Somerset region. Social Care Assessment Team The social care assessment team are a short-term service providing support to service users after discharge from hospital, working closely with the reablement service as part of the discharge to assess model. You will be based primarily in Peasedown St John, Bath but will work across the whole of Bath and North East Somerset region. First Response Team Is the point of contact for members of the public and professionals who require contact with Adult Social Care, and the person is either not known to the service or they are known but do not have an allocated worker with a Social Care Team. To apply you will have: A relevant social work qualification Knowledge of the legislative framework and legal proceedings that governs Adult services Knowledge of theory and practice applicable to social work IT skills to enable operation of standard electronic systems Passion, commitment, and motivation to support the most vulnerable service users in our area We are seeking applications from experienced workers or those newly qualified within social work and are looking to enhance and further develop their skills and contribute to service development for vulnerable adults.
