Responsibilities
Conduct comprehensive assessments of adults in need of social care services.
Develop and implement personalized care plans in collaboration with clients and their families.
Coordinate and monitor the delivery of services to ensure effective and timely support.
Provide ongoing support and guidance to clients, advocating for their needs and rights.
Maintain accurate and up-to-date case records and documentation.
Collaborate with external agencies and community resources to enhance service delivery.
Attend regular training and professional development sessions to maintain up-to-date knowledge of best practices.
Requirements
A recognized qualification in social work or a related field.
Experience working with adults in a social work setting is preferred.
Strong understanding of social care legislation and safeguarding principles.
Excellent communication and interpersonal skills to effectively interact with clients and stakeholders.
Ability to work independently and as part of a multidisciplinary team.
Proficient in case management and report writing.

End Time: 17:00
Pay Per Hour: £23.86
