An exciting opportunity has arisen for the role of a Head and Neck cancer Coordinator with in the Head and Neck department. The post holder will be the key person identified for providing cancer information and support, to enable people to navigate the health and social care system and make choices that are best for their cancer and their life. The aim of the role is to make sure that people living with cancer can access the right support, at the right time.
The post-holder will take an active role in contributing towards the development and Implementation of cancer survivorship initiatives enabling people affected by cancer to navigate the health and social care system and become self-supportive i.e. coordinating health and well-being clinics to enable independence and support self-management as appropriate.
The post holder will co-ordinate care by providing a single point of access for cancer patients including rapid re-entry into the system for those people identified as having urgent or specialist needs.
