About the Role: As a team leader you will take a key role in delivering the services within the home ensuring the promotion of good working practice and offering a role model to support staff. You will work closely with the Deputy Manager to provide professional support, supervision and guidance to Support Workers, and ensure continuity of care.
As a Team Leader you will be involved in monitoring and reviewing care plans in conjunction with the Deputy Manager, Care Managers/Social Workers, service users and their relatives. You will be take part in Co-ordination of the rotas for Support Workers and ensure continuity of care. Implement the Company's procedural requirements including Disciplinary, Health and Safety, Financial and Quality Assurance systems.
About the Service: Willow
Willow provides an intensive support service for up to eight individuals (over the age of 18) with Autism, learning disabilities and complex needs.
Residents may have particular difficulties with communication, challenging behaviours, physical health, epilepsy, and other well-being issues, which will be addressed with individualised behaviour support/care plans and risk assessments.
The service at Willow is designed to promote the well-being of each person to live a complete and fulfilled life through a person centred approach. The service follows a Person Centred Planning model so that each resident’s care and support is individualised to their particular needs. Each resident is encouraged and supported in many ways to play an active role in their own care and decision making process. Important elements of the service include the individualised activities programmes together with maintaining family contact and social networks.
