Are you an experienced Healthcare Support Worker, with a full clean driving licence and a car owner that is passionate about supporting patients in their own home in and around the South Manchester community. Are you enthusiastic, caring and have the experience to support patients discharged home from hospital? If so, this role is for you.

The Discharge to Assess service enables patients to receive safe, effective treatment at home following discharge from hospital. You will work as part of a team to complete support calls and implement therapy programmes. You will be involved in supporting the team in completing assessments and follow up visits over a 7 day period. You will receive regular training and supervision in order to maintain competencies.

In the role as Senior Nursing Assistant, working without direct supervision you will be responsible for delivering support calls and therapy plans. You will have access to a qualified staff member for advice and direction on care as required.

Communicating effectively with patients, relatives, carers, and other healthcare practitioners, ensuring any concerns are listened to and relevant actions are taken.

You will always act in a manner, which illustrates respect for privacy, dignity and confidentiality.

A full UK driving licence with access to a vehicle is required for this role as you will be travelling across the South Manchester Community locality.
