Radis Community Care have an exciting opportunity for a Band 6/7 Community Lead Nurse to join our team operating across the Central & West Midlands.

About the role
As a Community Lead Nurse, you will be undertaking care assessments and clinical risk assessments whilst providing advice and guidance to the wider business. You will be the clinical lead for complex packages in the areas covered, ensuring that the team of Support Workers, who you will lead and manage, are both theory and practically trained and up to date in conjunction with specific client care. The role also requires shared on call responsibilities.
You will be working as part of a wide MDT including commissioners, GP’s, family members and community professionals. The successful candidate will ensure that all issues relating to the safeguarding of vulnerable adults are reported according to policies and procedures.
There will be the opportunity to develop your own teaching skills and participate in staff education programmes and developing training profiles for each Radis Complex Care client. The role will include Maintaining, reviewing and auditing all client clinical documentation in the service and at the CQC/CSSIW Registered Office.
This role will involve some remote working opportunities.

Skills and Qualifications required for the role
We are looking for a self-motivated, professional Registered General Nurse with experience of delivering training and Ideally, experience with complex clients in a community setting. Excellent communication skills are required with a willingness to work toward PTLLS qualification. This role will involve some out of hours and overnight working when required. There will be travel involved over a large geographical area so a full driving licence and access to a vehicle is essential.
