Clinical pharmacy in general practice is part of an exciting programme of transformation which addresses our ambition to deliver person-centred, co-ordinated care across an integrated health economy.

We are looking for enthusiastic and forward-thinking pharmacists to complement the team. We welcome applicants with or without GP experience. If you have the right skills and qualities, we can provide on-going support to develop professionally. Experience of successful change management and delivering education and training is desirable.

The clinical pharmacy role within primary care can pro-actively help patients stay safe and well and out of hospital, as well as helping reduce the demands on general practice.



The role supports patients to get the best outcomes from their medicines by identifying and addressing medicines related issues, whilstconsidering both public and social care needs. You will work within a PCN, providing clinical leadership on medicines safety and quality improvement projects.



The role provides a great opportunity to join a committed team, as well as having access to national training through the Primary Care Pharmacy Education Pathway (PCPEP).

We are looking for candidates who are enthusiastic and adaptable to change, with good clinical and I.T. skills. Candidates should exhibit organisational skills and ability to communicate effectively with a wide range of healthcare professionals and other stakeholders including patients, families and carers.
