To work within the Discharge Team either in the complex discharge team or the discharge lounge.

To provide care to a group of patients in accordance with the NMC Code of Professional Conduct, Trust policies and procedures.

· Ensure clinical incidents and risks are reported.

· The post holder will be expected to work using evidence-based practice and research.

· Provisions of the Data Protection Act (1998) to maintain effective management of records ensuring information governance is always maintained.

· Comply with Trust Infection Prevention and Control policies to prevent harm to all.

· Use and update patient information utilising systems such as VitalPac and VitalFlo and PAS.

To perform procedures such as, but not limited to, ECGs, IV administration, cannulation, Venepuncture, male and female catherterisation as well as wound management (discharge lounge role only).


• To work within the Discharge Team either in the complex discharge team and the discharge lounge.
• To work closely with ward staff involved in discharge planning to ensure that the assessments and referrals for services relating to the patient’s discharge is completed comprehensively and in a timely manner.
• To support the established team in ensuring the delivery and maintenance of a high standard of care that is patient centred and evidence based.
• To work with teams and specialties across the system to facilitate safe discharges in a timely manner.
• Adhere to the NMC Code of Professional Conduct, Guidelines and Standards and ensure colleagues do too.
• Provide high standards of evidence-based patient centred care and knowledge for all patients, respecting dignity and privacy.
• Deliver high quality care for patients within the discharge process and ensure all discharge plans are robust to provide a seamless and cohesive transition to other settings.
• Attend mandatory training as a minimum.
