Clinical Nurse Specialist

Integrated Older Peoples Services

We are delighted to be offering an exciting opportunity to recruit to the Clinical Nurse Specialist role for Frailty and Older People within the Acute and Emergency care group.

This is a senior band 7 post working at the front door with the Acute Care of the Elderly medical teams and front door therapy teams to holistically assess the older frail patient using their knowledge of Clinical Frailty Scoring and acute care needs to ensure the correct clinical pathways are applied to achieve the best outcomes for this patient group.

You must be passionate about older peoples care and demonstrate a commitment to ensuring a positive journey is achieved by using your specialist knowledge and skills to establish the correct pathway either back out to the community or within acute care where indicated. Avoiding hospital admissions and reducing length of stays is an integral part of this role and your specialist assessment and follow up of the patient’s care will demonstrate the key performance indicators of this role.

You will be expected to work both autonomously and within a team and achieve your Non-medical Nurse prescriber qualification for which you can expect to be fully supported. This role will form part of the foundation for a further career pathway in older people’s services as the care group explore Advanced nurse practitioner nursing roles for the future.

Working within the Integrated Services for Older Peoples and Acute care of the Elderly (ISOP/ACE) Team, you will be responsible for delivering effective comprehensive assessment and management of a designated cohort of complex frail and vulnerable older adults in our acute care services (specifically in the Emergency Department and on the Older Peoples Assessment and Liaison and Frailty Units.
