• As a member of the dementia service the post holder will contribute to the delivery of patients’ individual care under the direction and supervision of the Dementia Clinical Specialist (DCS)
• Gather quality collateral history/ personal information from patients, families, carers and other relevant services (eg social worker, care agency)
• Deliver, monitor and embed DCS care plans for patients with dementia using clinical leadership skills to role model best practice to ward staff
• Work closely with dementia service colleagues and contribute to service goals, taking responsibility for managing and collecting and inputting data metrics
• Use clinical judgment to feedback to DCS patient progress and assist in setting patient goals that reflect their changing needs
• Ongoing carer communication/ liaison, including referrals to community resources where indicated (eg carer support organisations)
• Facilitate the provision of meaningful activities for patients, working closely with band 2s and volunteers
• Facilitate supported/ risk-positive discharges
