A nine month fixed term vacancy has arisen within the pulmonary rehab service to cover a period of maternity leave for a suitably qualified physiotherapist. Although there is no guarantee, there is a possibility of a permanent position at the end of the fixed term.

The post will involve assessment and management of respiratory patients referred to the pulmonary rehab service for exercise and education.

Currently pulmonary rehab is delivered face to face in a group settings (leisure centre based) or using a digital approach (online) as well as individually within the patient's own home (home delivery service).

The post will be based at Nevells rd health centre in Letchworth but there will be a requirement to travel extensively across East and North Hertfordshire to meet the needs of the service.

Full induction and training will be provided.

Assessment of patients prior to their attendance at pulmonary rehab

Formulation of individual management and/or treatment plans

Continuous assessment and evaluation of the pulmonary rehab programmes and modify according to the individual patient needs

Awareness of the needs of patients and their carers and to promote and encourage patient independence and where possible restore functional ability

Delivery of education to patients using online platforms such as MS teams as well as face to face in groups or individually

Key skills-exceptional organisational/communication/motivational skills are required.

Candidates should be familiar with Word, Excel and Powerpoint as well as common social media platforms and all common IT applications

You must hold a full valid driving licence and have access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

• To provide assessment and treatment for patients referred to the pulmonary rehab service (exercise and education).
• To act as a resource and provide advice for other members of the MDT.
• To develop and deliver an 6-week pulmonary rehab programme in a variety of non HCT sites (leisure centres) including new pathways developed in light of COVID 19 pandemic eg Online pulmonary rehab and pulmonary rehab Home Delivery.
• To maintain patient treatment records and ensure others do the same using System one.
• To undertake a clinical caseload to a high professional standard, using knowledge and skills from theoretical and practical experience.
• To be involved in the education and development of more junior staff to improve their skills and knowledge.
• To participate in clinical supervision to ensure that clinical practice is maintained to a high standard.

MAIN DUTIES and RESPONSIBILITIES:

Operational
• To undertake the assessment of patients using investigative and analytical skills and formulate an individual management and/or treatment programme.
• To undertake the continuous assessment and evaluation of the pulmonary rehab programmes and modify according to the individual patient needs.
• At all times to be aware of the needs of patients and their carers and to promote and encourage patient independence and where possible restore functional ability.

Patient / Customer Care
• To formulate and deliver individual management and treatment programmes based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills, patient exercise and education, and other rehabilitation options.

Management and Leadership
• To be responsible for the safe and competent use of treatment equipment by patients, staff and other services, through teaching, training and supervision of practice.
• To assist with the planning and implementation of induction programmes for new staff .
• To be responsible for equipment used in carrying out duties and to adhere to departmental equipment policy.
• To assist the record keeping of rehabilitation assistants according to Trust and professional standards; to ensure that all legal requirements are met.
• To participate in teaching others on professional skills and knowledge within core clinical areas.
• To teach, assess and contribute to the performance management of rehabilitation assistants.
• To take an active role in supervision and the appraisal scheme as an appraisee and be responsible for complying with agreed personal development programmes to meet standard of knowledge and competencies.

Communication and Relationship Building
• To act as the patient’s advocate throughout their rehabilitation programme and enable patient’s empowerment through the process.
• To communicate patient related information effectively. To ensure collaborative working with the multidisciplinary team members across health and social care sectors and all members of the Trust ensuring the delivery of a co-ordinated multi-disciplinary service.
• To use a range of verbal and non-verbal communication skills to communicateeffectively with patients to progress rehabilitation and treatment programmesand to ensure an understanding of their diagnosis so that patients are supported in self-management of their condition.
• To use communication skills of motivation, empathy, tact, explanation and gaining informed consent with a wide variety of patients.
• To overcome barriers to effective communication such as cognitive impairment, expressive and receptive communication difficulties, embarrassment, anxiety, pain, fear and visual/hearing impairments and develop and implement strategies to overcome these barriers.
• To use communication skills to address language and culture diversity issues.
• To maintain accurate and up to date documentation in line with legal requirements and communicate assessment and treatment results to the appropriate disciplines.
• To be able to communicate effectively with relatives and carers regarding patient care and progress with the consent of the patient involved.

Information Management
• To use SystmOne to maintain accurate patient notes on a daily basis and write reports when required.
• To achieve and maintain systems of information as requested.

Finance and Resource Management
• To be responsible for the replenishment of equipment and demonstrate an awareness of the value of equipment
