Key Responsibilities Deliver pre- and post-release support to individuals with health vulnerabilities, assisting with continuity of care and connecting them to community services. Conduct initial assessments and care mapping for service users, creating tailored care plans for effective release and reintegration. Organise and contribute to health promotion events, supporting the reduction of health inequalities and the promotion of wellbeing. Maintain accurate, detailed records in accordance with organisational and national standards, and manage multiple tasks to meet the demands of a high-volume, fast-paced environment. Qualifications & Skills NVQ Level 3 or equivalent in Health and Social Care or related field. Experience supporting vulnerable individuals to set and achieve goals, with knowledge of health, criminal justice, housing, and social care. Strong communication skills with experience in case management, structured interventions, and care planning. Proficiency in Microsoft Word, Excel, and database systems, along with effective time management and problem-solving abilities. Desirable:Experience within a prison setting or working with individuals involved in the criminal justice system, and additional mental health or coaching training.
