To work as part of the integrated team to deliver therapeutic input to patients with the aim of returning them to maximum independent living, within a stimulating environment actively promoting self-management on all activities of daily living.
Working as a specialist practitioner, the post holder will provide advanced and broad ranging diagnostics and case management of complex patients.
Work with colleagues across health, social care, mental health and voluntary sector to ensure holistic and joined up delivery of care.
Empower patients to make decisions about their own care and encourage them to exercise their rights and choices, signposting them to relevant community and voluntary services and supporting patients carers and members of the integrated team with management of complex presentations and risk.
Working as an autonomous practitioner you will undertake a differential diagnosis procedure i.e. you will distinguish a particular disease or condition from others that present similar clinical features and determine a specific diagnosis and subsequent treatment options
As this is a job share the current post holder works Monday, Tuesday and part of Wednesday, operationally we do require a candidate who can work Wednesday, Thursday and Friday. 8.30-4.30 or 9-5.
It is essential that you are a car driver with access to a vehicle for work as you will be expected to travel across the North East Essex area
To utilise advanced clinical expertise, function as an experienced clinician managing a specialised and/or complex clinical caseload within an integrated team
Act as a resource for the integrated team, carrying out single assessments, contributing to the development and delivery of the integrated care plan with a focus on patients with complex and specialist presentations
Develop programmes of specialist nursing/therapeutic activities, weighing options and delegating aspects of care across the integrated team as appropriate
Implement specialist aspects of the care plan, potentially acting as care coordinator and reviewing the plan on behalf of the integrated team.
To have clinical autonomy regarding the clinical diagnosis and future clinical management of presenting patients through clinical assessment and the requesting and interpretation of the results of specific medical investigations
Support patients with advice, information, persuasion and direction to community resources to encourage self-management and ongoing independence.
Provide support, training and advice to patients, parents and carers.
Direct the work of more junior staff, students, assistants and volunteers
Manage the work of the team, deputising for more senior managers as required
