Provide operational leadership, line management and co-ordination across Urgent Community Response (UCR) Services, linking with system partners to provide effective and efficient service delivery.

Support the development of creative solutions across traditional organizational boundaries to deliver the right care, in the right place, at the right time by the right person.

Play a key role in embedding a collaborative culture, developing bespoke and creative solutions to address patient’s needs and avoiding unnecessary hospital admissions.

Lead the promotion of the service to all potential referrers and stakeholders to ensure the service is accessed appropriately and having maximum impact, and to represent the team at appropriate engagement forums and meetings.



To lead and manage UCR services, supporting operational issues on a day-to-day basis.

To be responsible for high quality service delivery and foster a culture of interdisciplinary, multi-agency working to deliver bespoke care solutions.

Develop positive and professional relationships across a wide range of internal and external groups, agencies, and service providers.

Provide and receive highly complex, sensitive, and potentially contentious information, including presenting information about UCR to a wide range of internal and external stakeholders in formal settings.

Understand the whole health & care system to establish priorities for the service and address these on a constant basis to ensure high quality outcomes for the patient and system. This will be highly complex information and require analysis, interpretation, and comparison of a range of options to establish the right course of action.

To manage and analyse service activity and performance data to demonstrate compliance with service targets and key performance indicators, identify trends and key areas for service development.

To oversee the care of UCR patients, ensuring patients and carers receive the most appropriate care, delivered by the most appropriate person within the team, and they are discharged in a timely and safe manner.

To demonstrate, promote and encourage an understanding of positive health, and to promote the health and well-being of the patient and their carer, regardless of age, gender, ability, race, religion, or sexual orientation.

Champion the use of Assistive Technology and deploy it to support patient care and maximise independence.

Utilise risk stratification methods in partnership with other professionals to identify those patients most likely at risk of health, social care and wellbeing crisis and focus support to ensure the patient’s independence is maximised and goals set to achieve positive health outcomes.

Liaise with GPs, Social Services, inpatient teams, other allied health care professionals and the voluntary sector to ensure identified needs are met and care co-ordinated appropriately.

Demonstrate leadership skills, challenge own and others practice, behaviours and beliefs in ways that facilitate change and improve services.

To identify and minimise any barriers to effective communication within the team and patients/carers served by the service and be responsible for ensuring workload for self and others is planned and prioritised according to service targets and needs.

Lead the multidisciplinary team meetings and other service specific meetings where required and develop and maintain effective working relationships and communication with all members of the organisation, other professionals, and agencies.

To monitor and oversee allocated budgets for direct reports, ensuring appropriate use and balance of funds and to be an authorised signatory for goods, services and payroll.

To monitor and oversee staff absence, annual leave and study leave in line with NCH&C’s policies, procedures and guidelines.

Undertake training as required including compliance with mandatory training and maintain and participate in own appraisal and personal development plan as well as undertake personal development plans for more junior staff members.

Be responsible for safe use and maintenance of equipment and supplies.

Remain accountable for own professional actions as determined by the professional bodies.

Participate in evidence-based practice, using research methodology.

Develop and implement national and local policies and pathways to ensure the effective delivery of the service, proposing changes and advising other external agencies about these changes or developments.
