This is an exciting opportunity to lead a community Parkinson's specialist nurse service in Central and South Manchester. You will be working in a highly autonomous role alongside the community neuro rehab teams, other specialist nurses, consultants and GPs.
Operationally the service sits within the Central Manchester Community stroke and Neurorehabilitation Team portfolio but also provides a service to South Manchester locality patients , and therefore has close links to the Neuro rehabilitation Pathway, which includes Physiotherapy, Occupational therapy, speech and language therapy, Clinical psychology, and a stroke and Neurorehab Nurse in each locality.
Flexible and part time working options could be considered.
The Community Parkinson's Specialist nurse main duties will include:
• Holistic assessment and prescription of complex management plans for people with Parkinson's either as part of routine condition reviews or in the presence of an identified deterioration
• Support patients and their relatives when newly diagnosed with Parkinson's
• Provide other health professionals with specialist advice, training and support in the management of Parkinson's patients in their services.
• The post holder will be responsible for driving service review and development, and producing regular service reports for senior leadership.
