The post holder is an experienced general practice pharmacist, who acts within their professional boundaries, supported and working alongside a team of GreenLight pharmacists in general practice. In this role they will be supported by practice GPs and the GL@GP Director who will develop, manage and mentor them. The post holder will work as part of a multi-disciplinary team and provide clinical pharmacy leadership for the practice, taking responsibility for, and not limited to, Structured Medication Reviews, areas of chronic disease management, medicines optimisation, quality improvement, the quality and outcomes framework and other enhanced services. The post holder will be responsible for leading, training and coordinating the work of less experienced pharmacy professionals to proactively and competently manage repeat prescribing systems, deal with acute prescription requests and medicines reconciliation on transfer of care, if necessary putting in place systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patients in the GP practices. The post holder will provide support to practice staff with regards to prescription and medication queries, ensuring integration with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage GP workload. The role is pivotal to improving quality of care and operational efficiencies and so requires motivation and passion to deliver excellent service within general practice.
