Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here.   • Evidence of conduct in previous employment concerned with the provision of services relating to. Health or social care or Children or vulnerable adults

The transitions planning team supports young people to prepare for adulthood, working closely with those people who are important to them, along with statutory services covering health, education, children’s social care and the voluntary sector. We recognise that preparation for adulthood is a time of celebration, change, choice, decision-making and challenge for all young people.
You will be working with the young person and their support circle whilst also ensuring that the statutory requirements under the Care Act are met. You will work across Manchester and sometimes beyond, depending on where the young person lives to lead the transition from children's social care to adult social care.




We are looking for a qualified Social Worker to join our team to ensure our young people are prepared for adulthood and supported appropriately into independence.

To do this you will need to be:

  • Person centred and strengths based.

  • Able to develop good working relationships with young people and their support network.

  • Open to developing your skills to offer the best outcomes for our young people.

  • Able to work with complex scenarios that may require an innovative response.

  • Able to collate and draw on evidence, as well as current research, to support your work.
