Service Care Solutions are looking for a Key Worker to work within the Cheshire East Council on a 3-month contract.

Job role/Responsibilities:
Undertake statutory assessments and develop and maintain EHCPs across the Local Authority for children with SEND. Manage a caseload of children with EHCPs, attending annual reviews and updating plans as needed. Liaise with childcare, education, and vocational providers to secure appropriate placements for children and young people with EHCPs.
- Coordinate a person-centred, outcome-focused, multi-agency approach to assessment, planning, and support, ensuring young people and parents are treated as partners in creating EHCPs. Act as key worker for allocated young people requiring SEND assessments or EHCPs.
- Serve as the Named Officer and primary contact for assigned schools, colleges, and institutions attended by Cheshire East Council residents with SEND.
- Attend and, when necessary, chair multi-agency co-production meetings. Analyse and evaluate educational, medical, social care, and psychological reports to make balanced, outcome-driven service recommendations.
- Meet with young people, parents, and carers to discuss and resolve SEND support decisions, guiding families to Independent Advice and Support or advocacy as needed.
- Submit comprehensive evidence to Moderation Panels to facilitate decision-making.
- Support the SEND team in promoting an inclusive approach, advising families to make informed choices about special educational needs support.
- Represent the Local Authority in mediation and appeals, including First Tier Tribunal (SEN and Disability).
- Track progress and outcomes of commissioned provision for young people and ensure achievement of outcomes.
- Liaise with non-educational agencies to secure necessary services and advise schools to effectively use SEND funding.
- Ensure resources for SEND support and EHCPs are allocated, monitored, and reviewed, including guidance on personal budgets and outcome measures for reporting young people's progress.
Knowledge/Expereince required:
- Extended experience in/with early years, school, college, Local Authority, health, independent setting in relation to children and young people with special educational needs and disability.
- Successful experience of collaborative and partnership working with school staff and partner agencies.
- Effective working with families or young people.
- Effective multi agency working Statutory Assessment (SEND, Child Protection etc.).
- Reviewing monitoring and evaluating performance against agreed targets. Presenting evaluative reports to ley and professional audiences.
- Working with and delivering support//careers guidance/advisory services to children /young people and their parents.
- Effective up to date theory and practice in the field of special educational needs relating to areas of responsibility.
- Understand relevant legal frameworks.
- Awareness and understanding of the impact on parents/carers.
Qualifications:
- Graduate level qualification in Education, Social Work, Health-related field or Psychology.
- And/Or NVQ Level 4 Advice & Guidance
- And/Or NVQ L&D, Support Services for Children
