Be responsible for the Childrens Continuing Care assessment of children and young people with Mental Health/ LD needs and Physical Health needs. In conjunction with lead provider and partner agencies e.g., Physio, OT Have strategic oversight of complex discharges from tertiary and acute centres. In conjunction with the Case Manager, build strong links with lead provider other agencies, e.g. the Childrens Centre, Education Health and Care process, social services and specialist educational schools ensuring that families receive the most appropriate and equitable service for their Initiate co-production to ensure children/families are involved in the planning and developing of their own Ensure robust processes for triaging of referrals, DST assessments and reviews as well as escalations for CCC framework implementation.
