Job Title - Home Help Coordinator

Location - Community Engagement Centre, Ford Street, Walsall, WS2 9BW

Salary - £13,281 per annum - £12.14 PH

Contract type - Permanent

Working hours - Part Time 21 hours per week (over three days to include Friday)

About the role



The role is based within our Health and Independence Service in Walsall.



We are looking for an individual to join our team, as a home help coordinator. Our

Help at Home service supports older and vulnerable adults to remain independent in their own homes. The scheme provides practical help around the home such as shopping, cleaning and ironing, alongside social interaction and companionship for service users across Walsall.

The role will involve providing admin support to the service including, rotas, inputting data, initial assessments and reviews. Wider admin support will be required for the health and independence services. In addition the successful candidate will demonstrate their ability to carry out practical tasks such as cleaning to a high standard, to develop relationships with clients and offer companionship and friendship, and the qualities of reliability and trust which a role working with our client group requires, when a cover call is required.

We are looking for someone who is able to:

Ensure that an accessible support service is made available Coordinate and facilitate the day to day rotas for the service Work alongside the Project Manager and our other services, to provide good quality home based services, ensuring best practice principles are adopted and applied. Provide service delivery cover as and when required Develop and maintain service literature, ensuring information is available and accessible for potential service users, their carers and key stakeholders. To actively publicise and promote service across the borough of Walsall Ensure that staff individual Training and Development programmes are completed. Assist with the development and implantation of company and scheme policies, procedures and processes.
Day-to-day operational responsibility to ensure the smooth running of the service, ensuring all calls are effectively covered. Day to day organisation of services, ensuring priorities are identified and met efficiently and effectively, making good use of all available resources.
To provide cover for any essential calls this will include, cleaning, shopping or accompanying to appointments. Respond in an effective and timely matter to new enquiries and referrals as set out within the Help at Home procedures. Ensure that clear, accurate and timely records are maintained both on the database and client file in respect of all client support and business activities.
Conduct assessments of risk with particular reference to safe practice in relation to home visits Organise regular spot checks on the work delivered out in the community together with Quality Of Service Questionnaires and ensure that information is acted on where necessary, keeping line management informed. Produce progress reports as required and ensure that accurate information is communicated Promote a health and safety culture in the workplace Manage personal resources and own professional development Undertake other activities as required
The post will have a working base at our Community Engagement Centre, Ford St., Walsall, WS2 9BW, but the post holder will be expected to work across Walsall at a number of different locations.Therefore a clean driving licence and access to own transport is essential.
