The post holder will be required
• To take a leadership role in the effective provision of rehabilitation within Pembridge Palliative Care Centre as a senior member of the multidisciplinary team.
• To undertake highly specialised physiotherapy assessments of adult patients with specialist palliative care needs who may have a highly complex presentation and to produce specialist individual treatment plans.
• To be instrumental in developing the organisation’s rehabilitative approach to palliative care.
• To provide highly specialised advice to Hospice and other staff to facilitate the management of patients in on-site and community services.
• To work effectively with all disciplines, agencies and relatives involved with the patient. To communicate with these agencies, relatives and carers in an appropriate and sensitive manner.
• Prioritise and manage own workload with minimal supervision.
• Work within professional standards and clinical guidelines, promote best practice and to undertake all aspects of clinical duties as an autonomous practitioner including assessing and treating own caseload of patients/clients and maintaining professional documentation.
• Be responsible for the supervision of therapy assistants and students on placement.


• Undertake the comprehensive, specialist, physiotherapy assessment of adult patients with progressive and life-limiting diseases, including those with highly complex needs, using investigative and analytical skills and outcome-measurement tools, and to discuss assessments with other team members;
• Plan, implement and evaluate specialised individual physiotherapy management/treatment plans using clinical reasoning skills and in conjunction with patients’ own goals for care and relatives’/carers’ input;
• Provide holistic care with attention to patients’ multifaceted needs including physical, emotional, psychological and spiritual;
• Be highly skilled in communicating with and supporting terminally ill patients, their families, formal and informal carers;
• Be a source of physiotherapy/rehabilitation expertise providing clinical leadership skills across the multidisciplinary team to encourage an organisation-wide rehabilitative approach to patient care;
• Provide a specialist physiotherapy service to optimise non-pharmacological management of symptoms including developing the service to include symptom-specific groups;
• Provide specialist advice, teaching and instruction to relatives, carers and other health and social care professionals both within the organisation and externally, to ensure a consistent approach to the patient's care;

Our temporary staffing shifts are calculated hourly and paid weekly. You are paid only for the hours you actually work during any Assignment. Bank workers are paid on fixed third point based on the Agenda for Change (AFC) pay bands.

· Be aware of, and act in accordance with CLCH’s clinical, operational and organisational policies and guidelines.

· Ensure that policies, procedures, clinical governance arrangements and lines of accountability are adhered to by the therapy assistant to ensure safe and effective clinical service delivery.

· Ensure the timely and correct entry of clinical and statistical data onto SystmOne.

· Complete statistical returns as required by CLCH including entering data onto the computerised systems within the required time frames.

· Actively participate in clinical supervision.

· Facilitate clinical supervision on a regular basis to designated staff members.

· Participate in an annual appraisal and maintain a personal development plan.
