An exciting opportunity has arisen for a motivated and enthusiastic Registered Nurse with current NMC registration to join a dynamic out of Hours District Nursing Team covering the city of Salford.
We are looking for a group of highly motivated applicants who are interested in the challenge of providing holistic care management, through assessments and evaluation of health needs in the community with patients in their home setting. You will be working with a variety of disciplines within the multidisciplinary team to achieve the best outcomes for patients to ensure their independence and wellbeing. The service delivers essential care to prevent hospital admission.
The evening and Overnight District Nursing Service is a diverse team delivering quality assured nursing care to those aged 16 and above living in the city of Salford, ensuring that patients and their families/carers maximise their independence and remain in their own home.
This highly skilled and dynamic citywide out of hour’s District Nursing Team operates a 7 day service from the hours of 19.00-08.00, including weekends and bank holidays.
The core hours of the post will be either;
2x evenings from 19:00-00:00 and 1x evening/overnight 19:00-08:00,
or
1x evening from 19:00-00:00 and 2x evenings/overnight from 19:00-08:00.
Over 7 days per week.
Flexibility will be required.
Due to the nature of this post it is essential that you will have a full clean driving license and will have regular use of an appropriate vehicle (as determined by the Trust).
