1. To provide support to service users in our female only service, as outlined in their care plans, person centred support plans etc to enable them to lead as independent lifestyle as is compatible with their needs and in accordance with their aspirations.


2. Using Person Centred Planning to develop and maintain support based on the identified needs and individual aspirations of the tenants. To ensure that service users make use of opportunities in the community for a meaningful and independent life to maximise their full potential. To work to ensure compliance with all regulatory requirements. 


3. With the support of the service manager to assess suitability of prospective tenants into the supported living scheme using Explicit Care Solutions (ECS) eligibility criteria in liaison with Adult Social Care Departments and other key stakeholders, ensuring equal opportunities. 


4. Where the property is owned by an independent landlord, ensure the service users are supported to maintain their tenancy agreements and that the property remains health and safety compliant. 


5. To liaise with Care Managers and other professionals to identify the changing needs of tenants and to access appropriate services as required. 


6. Carry out risk assessments and ensure support plans are in place for tenants. To promote a balance between the expressed wishes and choices of service users whilst ensuring that tenants do not expose themselves to uncalculated risks or harm. 


7. To provide or find information, advice and guidance to service users, or support in accessing the appropriate services to do so, on matters relating to finance, good housekeeping, health, neighbours, safety, security and daily work and leisure activities and ensure that this is done in a way that respects dignity, privacy and choice. 


8. To support service users in accessing local services and resources in the community in order to develop independence and fulfilment in all aspects of life. 


9. To respond flexibly to emergencies, this may involve arranging appropriate assistance or attending to problems out of hours etc. 


10. Attend ECS meetings to ensure participation in forward planning and continuity of Services and Quality Standards. 


11. To comply with ECS policy and procedures as indicated in the staff and centre handbooks and other ECS literature. To ensure that you are familiar and working within ECS policies and procedure. 


12. To ensure that you have understood the ECS equal opportunities policy and to ensure that duties are carried out within the policy and best practice. 


13. To undertake such training and development as the job may require.

14. To carry out any other reasonable duties as directed by your manager.
