Are you what we are looking for? Do you consider yourself to be the next generation of leaders in the NHS, If so an exciting opportunity has arisen for a Band 6 Nurse in the Outer North West Division for a community nurse to work closely within the planned care of Nursing and Therapy.

This is a permanent post.

This is a great opportunity to develop your understanding of quality drivers and to work closely with clinical teams to support the delivery of high-quality care and positive patient and staff experience.
We are looking for a dynamic and motivated individual with nursing experience who can provide local clinical leadership for the multi-disciplinary team working across the Division.


The successful candidate will have the opportunity to further develop quality expertise and lead key projects and pieces of work by developing strong relationships and network across the Division. You will work collaboratively with the wider multi-disciplinary team, other health and social care practitioners and the nursing staff, to provide the best care and
quality outcomes for patients and service users, in-keeping with national guidelines.

The post holder will work in collaboration with the designated Deputy Team Leader in managing the care of patients with long term conditions, incorporating monitoring progress and promoting the preventative aspects of nursing practice.

Job Purpose
• To provide skilled nursing care working as part of Central London Community nursing service to
patients in a variety of community settings between the hours of 0830 and 2100 hours, including
weekends.
• To actively monitor, maintain and develop the service provided.
• To take responsibility for managing the caseload and the team in the absence of the DN team lead.
• To deliver a case managed approach for patients with long term care needs.
• To actively participate in service development activities
