PURPOSE OF JOB POSITION:
As a Community Care Worker, you will be responsible to look after the physical, emotional, cultural and social needs of the Clients using a person-centred approach. You should observe and promote the patient’s choice, independence, dignity, privacy, fulfilment and other rights and maintain good professional relation-ships with Clients, their family and friends and other stakeholders. You should actively support other Care Workers, care professionals in delivering safe care whilst adhering to all regulatory and statutory obligations, Policies, Procedures and Guidelines.

PRINCIPLE DUTIES OF THE JOB ROLE INCLUDE:
• To provide personal care and support to every Client with a wide range of needs, illnesses and disabilities;
• To know and understand the care and support requirement of the Clients;
• To undertake the tasks detailed in the Clients’ care and support plan using a person-centred approach and in the least intrusive and restrictive way;
• To encourage self-independence and self-motivation of the Clients and not foster dependent behaviours;
• To provide input into the care and support of Clients by regularly feeding back to the Field Care Supervisor/Care Coordinator and management team.
• To assist Clients getting up in the morning and going to bed at night;
• To assist Clients with all their personal care needs such as washing, bathing and shower, dressing and undressing, looking after their skin integrity such as pressure sores, wounds and ulcers, oral care, toileting and continence management, grooming.
• To assist Clients with their medication at the agreed level of support and as detailed in their Medication care plan
• To prepare food and drink for Clients, being aware of clients’ choices, likes/dislikes, nutritional needs and cultural requirements;
• To provide light general household domestic duties, including housework and laundry, as agreed and detailed in the care plan or instructed by Management;
• To use manual handling equipment safely and correctly for service transfers
• To take responsibility for the safe handling of property and equipment belonging to clients.
• To maintain good communication and develop effective working relationships with Clients;
• To provide companionship to Clients, actively talking and listening to them about their interests;
• To help the service users to maintain contact with their family, relatives and friends;
• To accompany the Clients on trips into the community e.g. medical appointments, shopping, and other social outing activities
• To assist the Clients to manage their personal affairs;
• To ensure as safe as possible the living environment for service users, whilst respecting Service users’ choices and rights.

GENERAL RESPONSIBILITIES : POLICIES AND PROCEDURES
The post-holder is required to familiarise themselves with all policies and procedures and to comply with these at all times. Failure to comply with any of these policies may result in disciplinary action up to and including dismissal. As required maintain accurate, legible, daily care records and submit them. Participate in Foxbridge’s choice of Electronic Call Monitoring system as appropriate. Always maintain confidentiality. Advise Service Users of the complaint’s procedures (where appropriate). Participate in all training courses, including professional development training such as NVQ in Health and Social Care. Attend and participate in team meetings as appropriate and attend regular supervision and annual appraisal sessions. Collaboration with other Health & Social Care Agencies.
Assist the management to maintain good working relationships with clients’ families and other Health and Social care professionals involved in the care of the clients, as part of a caring team.
Carer Must be able to work at least 20hrs per week. Job post ONLY suitable for a person who drives and owns a car that can be used for work. Hourly rate include car meleage allowance.
