About this role…

We are looking for a support worker to join the North Living Well Centre team.

The Lambeth Living Well Network Alliance supports people in Lambeth experiencing mental illness. Thames Reach works in partnership with South London and Maudsley NHS Foundation Trust, Lambeth NHS Clinical Commissioning Group, Lambeth Council and Certitude to deliver mental health services as part of the innovative transformation of mental health services in the borough.

We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions.

You will:

.Be a capable and enthusiastic support worker
.Be joining the North Living well Centre, a service which is part of the innovative reshaping of Mental Health services in Lambeth.
.Work directly with clinical staff to manage a caseload of people with Mental Health needs in the Lambeth Community, focusing on resolving practical issues and improving their wellbeing and social integration and supporting them the to lead independent lives in the community alongside their GP.
.Work with Staff in the Living well centres supporting clients using mix of brief interventions or longer-term support as needed, delivered by multi-disciplinary teams consisting of support workers, community psychiatric nurses, social workers, occupational therapists, peer mentors and consultants.

To succeed as the Support Worker you will have:

.Knowledge of the needs and hopes of vulnerable people – particularly those with mental health issues and complex needs.
.An understanding of the issues facing vulnerable people and the services they require to address these.
.Experience in working quickly and building relationships in a short period of time.
.Experience in working in partnership with external agencies and health & social care professionals.
.A good understanding of social support needs including the difficulties of social exclusion and housing issues.
