Here at our Continence service no day is ever the same. As a well established Community Service, we care for patients with complex needs providing highly skilled assessments and support within their own home environment and in a clinic setting.

The role will involve working within a team of experienced Community Continence Nurses and working in partnership with multiple stakeholders and voluntary services to provide the best care to our community and shape the future of healthcare across Milton Keynes.

We are looking for nurses who can think on their feet and thrive on the challenges the working day can bring. You will be joining our team of highly motivated proactive professionals to continue our goal of developing our service beyond the traditional scope in response to the challenging health care needs that we are facing nationally.

The hours are 9am – 5pm, 5 days a week. Driving licence and use of a car is essential.

• The service sees adults with bladder and or bowel dysfunction with or without incontinence. The post holder will take a key role in the delivery of the service including assessment, development of individualised treatment plans, give advice to patients, carers, healthcare workers and care home staff, both in clinic settings and at domiciliary visits.
• Takes responsibility of new visits/assessments, workload management and allocation, dealing with referrals,
• Maintains close liaison with the District Nursing teams, GP and other members of the primary care team and wider health and social care.

• Encourages patients towards self care
