EXCITING OPPORTUNITY FOR A SENIOR COMMUNITY NURSE IN NORTHKENSINGTON & CHELSEA COMMUNITY MENTAL HEALTH HUB

We are delighted to offer the opportunity for an experienced community nurse to join our nursing team as a band 7 Senior Nurse.

The post will be based at North Kensington & Chelsea Community Mental Health Hub based at Hathaway House. This would be a great development opportunity for band 6 RMN’s with community experience. Be part of a vibrant, supportive team whilst developing leadership skills. This will be an exciting opportunity to make a difference in community nursing, and ultimately contributing towards patients care.

The CMHH is an exciting area to work in as it is at the forefront of mental health transformation in line with the five year forward plan. To be considered for this post you will need to be a registered mental health nurse and be keen to develop experience of working in multi-disciplinary teams who care for adults within mental health secondary care sector.

The post holder will work as a Clinical Team Leader in the Borough team providing brief clinical and social care intervention for service users.
• To Triage referrals and co-ordinate duty assessments.
• Be responsible for the supervision of staff & their case load.
• Facilitate appropriate clinical practice of the team including CPN’s, OT’s, Administrative staff and AMHPS.
• Draw up the monthly rotas
• Develop and maintain close professional relationships with GP’s and other professionals.
• Ensure staff practices in a safe, appropriate and professional way with service users and their carers.
• Attend professional practice forums
• Organise and investigate Safeguarding alerts in line with local Safeguarding policies.
• Chair clinical and team meetings as appropriate.
• Provide senior support an cover for the Team Manager
• To assist the team to be compliant with KPI and monitor electronic records

• To carry out needs assessment of individuals presenting to the duty service in crisis.

• To identify vulnerable adults and provide effective programmes of support that protect and promote their health and wellbeing, intervening appropriately when a person’s safety or welfare is at risk.
• To ensure that CNWL Trust and LA protocols are adhered to by team members. · To cover for the absences of senior colleagues as appropriate and as required by service needs.
• To contribute to the implementation of the Clinical Governance agenda including Target Setting for Teams, Clinical Audit.
• Participate as a key member of the Borough Service to ensure collaborative and innovative practice.
