Kingston Crisis Resolution & Home Treatment Team (KCR&HTT) is a multidisciplinary, community based mental health team that aims to provide a safe and effective home based assessment and treatment service as an alternative to in-patient admission, and for early discharge from the ward in the least disruptive and restrictive environment. It works in conjunction with, and is integrated with, other local service elements. The workload is team-based and team members do not carry a specific caseload.
The Multidisciplinary team is made up of Nurses (RMN), Recovery Support Workers (RSW), Occupational Therapist (OT), Team Administrator, Peer Support Worker (PSW), Advanced Clinical Practitioner (ACP), Line Manager, Consultant Psychiatrist, Trainee Doctors and Nursing Students.
The Team is available to provide care and intervention from 08:30 to 22:30, 365 days of the year for people who are experiencing a mental health crisis of a nature or degree that would require admission in the absence of the CR&HTT.
The post holderMUSThave a valid driving licence and access to a car to enable them to complete home visits. Please do not apply if you do not fit these criteria.
The post holder will be based at Tolworth Hospital and work across the borough of Kingston. There is an expectation that the post holder is able to work shifts.
Early Shift: 08:30 to 16:30
Late Shift: 14:30 to 22:30

About our locations:
Tolworth Hospital, Surbiton
To create the very best environment to care for our patients, this site’s layout will be transformed in 2026 to add a number of wards to the existing three. Tolworth has a canteen and is near shops, the A3, Tolworth station, and is a bus ride from Surbiton station, with fast connections to Clapham Junction and London Waterloo.
Under the supervision of Nursing Staff, to provide direct physical and psychological care to clients through participation in the care planning, provision and evaluation of client needs whilst ensuring a high standard of nursing and social care.
• To work effectively alongside Nursing Staff as directed by the Shift Co-ordinator and/or Team Manager in the following areas:
• To work in partnership with the client across boundaries of care, organisation and role, coordinated through the Care Programme Approach.
• To provide support directly and flexibly to meet the specific needs of clients in accordance with their care plan whilst promoting Social Inclusion, Rights and Responsibilities.
• To work as part of a team promoting Recovery in a way that:
• Promotes hope, helps people to take control and access the opportunities that they value;
• Engenders empowerment and well-being;
• Acknowledges diversity;
• Promotes anti-discriminatory practices;
• Maintains confidentiality;

Key Objectives / Responsibilities of Post Holder:
• To hold a valid Driving Licence and have access to a vehicle to enable travel between the Team Base currently located in Wallington and client homes.
• To work closely with the Nursing Staff/Shift Co-ordinators.
• To assist in the assessment of clients’ needs, risk and the implementation of individual plans of care.
• To support clients with return to the community after a period as an inpatient, in relation to practical arrangements and emotional support.
• To develop rapport with clients within appropriate boundaries based upon professional support and companionship.
• To undertake direct client work where this is deemed appropriate, and to actively engage clients in working to achieve the outcomes sought through their care plans, including the maintenance of contact with mental health services.
• To provide practical and emotional support to clients and carers to maintain dignity and independence.
• To positively promote and support independent living for clients in the community by maintaining extensive knowledge and links with the community resources and actively supporting clients to use them.
• To assist clients in signposting them in accessing resources in community i.e.-benefits and welfare rights, legal advice services.
• To help clients access information on health promotion.
• To identify signs of relapse and increasing risk by monitoring the client’s progress, level of functioning and mental state and informing the Shift Co-ordinator or other staff involved in the care of the client.
• To provide practical support in emergency situations as required (i.e. accessing emergency food provision, arranging protection of property, transporting, arranging services, arranging protection of pets, collecting prescriptions, clothing, assisting with homelessness and seeking accommodation).
• To contribute to the management of risk and the protection of individuals from abuse.
• To provide support and safeguard individuals who are at risk from abuse.
• To identify trigger points to reduce and respond to risk and violence. Protect yourself from levels of risk and violence in your own work environment.
• To attend and actively participate in team meetings/care plan review meetings and team training as appropriate.
• To maintain accurate records as per legal policy, existing procedures and protocols.
• To record and document as required (and after appropriate training) physical observations, such as weight, temperature, pulse and respiration and blood pressure. Report to the shift co-ordinator any significant findings.
• To receive telephone calls and record messages ensuring they are passed on to the relevant person.
• To work as a team member to promote and maintain positive communication and joint working practice to develop skills, knowledge and own safe and systematic practice, in line with joint working practice, policies and procedures.
• To support fellow Recovery Support Workers in their role providing advice/mentoring/training where appropriate.
• Participate in supervision and appraisal processes to enable reflection and personal development.
• Be aware of one’s own limitations in terms of experience and knowledge. Make this known if asked to perform duties beyond one’s competence.
• Participate in the planning of protocols to develop the service to the clients.
• Participate in the review of the service provided to clients overall by Recovery Support Workers and the team as whole
• To liaise and work in close co-operation with other mental health professionals, voluntary and private sector agencies as required, helping to ensure that best value services are delivered to clients.
• To be aware of and adhere to agreed procedures for delivering care. Report to the shift coordinator / Team Manager any areas of concern, which may affect the safe delivery of care and/or the safe working of other members of the team.
• To report any accidents / incidents as per the reporting procedures of the service.
• To flag up disciplinary or performance issues with appropriate line manager.
