The service sees adults with bladder and or bowel dysfunction with or without incontinence. The post holder will take a key role in the delivery of the service including assessment, development of individualised treatment plans, give advice to patients, carers, healthcare workers and care home staff, both in clinic settings and at domiciliary visits. Takes responsibility of new visits/assessments, workload management and allocation, dealing with referrals, Maintains close liaison with the District Nursing teams, GP and other members of the primary care team and wider health and social care. Applies current Infection Control Policy issues and universal Encourages patients towards self care
