An exciting opportunity has arisen to join Brent Community Rehab’s Discharge to Assess Team. We are looking for physiotherapists to work closely with Brent Adult Social Care colleagues working with Reablement to deliver world class reablement to Brent residents. This is a great opportunity to experience truly integrated working in a supportive environment to provide high quality patient centred care in the community.

Applicants should have a relevant professional qualification and clinical experience working in community rehab.

You will be provided with learning opportunities, support to develop your skills including clinical supervision, appraisals and training, as well as career development.

You will have access to the Trust’s Academy and professional networks to access courses and meet your identified development needs.

Undertake specialised uni-professional and multi-professional assessments.

To develop and implement treatment plans for clients. These clients will present with primarily neurological conditions, but you may also see musculoskeletal conditions, osteoporosis, learning disability and/or mental health diagnosis.
