Mk Together is a collaborative partnership in Milton Keynes tasked with providing an integrated Health and Social Care offer for the people of Milton Keynes. This partnership brings together Milton Keynes University Hospital Foundation Trust, Milton Keynes City Council and Central and North West London Community Health Services.



MK Together is working to break down organisational boundaries to enable us to better provide Health and Care services for people in MK.



This exciting project aims to improve the patient experience in providing seamless transition between hospital and home and Home 1st therapy now require a team of enthusiastic and proactive therapists to work across our hospital and community service to support people in getting home safely and quickly to prevent the complications of experiencing long stays in hospital.

The integrated discharge hub based at Milton Keynes Hospital, is responsible for being a single point of contact for professionals, providers, patients and their families to support safe and timely discharge from hospital. The ethos of the hub is to provide a seven day service to ensure patients are central to all discharge planning and they, along with their families are kept updated through their discharge journey and seamlessly returned to their home/chosen residential setting.



The hub is responsible for the safe and timely discharge of patients, reducing delays in discharge by coordinating professionals to ensure the necessary support and services are in place.

Here at Home First, we pride ourselves in the ongoing development of staff, we have successful applicants on ACP and Apprenticeship courses for both Physiotherapy and Occupational Therapy and have regular students on placement with support from our Practice educators.

Working closely with our Professional Lead Therapists and Team Leads, the continuing development of the academic learning and vocational skills of the therapy team is imperative. Your participation in staff supervision, training and completion of competencies and observation of practice is key to the provision of the ongoing standard of care we expect at CNWL.

You will receive a comprehensive induction, followed by a tailored development plan, which includes self-directed learning and collaborative working within Home1st and its partners.

But it doesn't stop there . We can offer coaching and opportunities for daily debriefs, access to remote working, regular 1:1 formal and informal supervision, peer group CPD sessions and in-service training, QI project work and volunteering for champion roles.
• To ensure communication is clear and carried out in a way that consistently displays professionalism at all times to ensure effective working relationships with colleagues, clients/patients and other professionals/agencies.
• The post holder will communicate in person, via telephone calls, emails and MS Teams and ensure information is provided and received in line with GDPR and Information Governance. This will require high levels of interpersonal skills and a level of tact and empathy. This will also require advising and signposting to appropriate services.
• Provide a presence and support to designated wards/departments to facilitate safe and timely discharges from hospital including attendance at board rounds to partake in setting PDD and identify and resolve any barriers to achieving PDD.
• To report daily into the IDH management of updates on patients discharges.
• Attend meetings/huddles for allocation of patients to commence discharge planning.
• Provide Information and support to patients, families and staff in relation to complex patients discharge from initial contact to 72 hours post discharge
• Lead and/or participate in MDT meetings, discharge planning/professional and best interest meetings to support patient discharge
• Work alongside the other members of the IDH to promote the IDH’s role in discharge planning. This involves educating and empowering other professionals to be involved in discharge planning.
• Work with the MDT to implement discharge requirements for each patient on admission to the ward and ensure early referrals.
• Recording all information regarding the patient in the electronic patients records.
• To ensure that there is discharge practitioner representation during the operational hours of the IDH
• To manage complex and sensitive information on a daily basis, using empathy and reassurance skills at all times with all people. Ensure that information is recorded and shared in line with information governance.
• To contribute to the rota management.
• Ensure relevant databases used in discharge planning are updated to enable accurate oversight of patients length of stay and any barriers to discharge.
• To contribute in any system events related to patients discharge and length of stay (e.g. escalation).
• Ensure that information relating to a patient’s discharge is accurately recorded and kept secure on the appropriate database/system and the electronic care record.
• Ensure that the referral to IDH is completed in full as soon as possible after admission, for patients needing or requiring support with discharge by the appropriate professional.
• To collaboratively work with the patient and/or representative along with MDT professionals to set goals for discharge and beyond.
• Ensure that at board rounds the MDT sets the PDD as per national golden standard (within 48 hours of admission) and identify and resolve any barriers to achieving PDD
• To lead on complex discharge planning working with the MDT to ensure discharge is safe and seamless.
• To resolve barriers in relation to a complex discharge and escalate further as and when necessary in order to facilitate safe and efficient discharge.
• To work collaboratively with and delegate or share duties as appropriate to other members of the IDH to coordinate complex discharges.
