The team provide an urgent response and a comprehensive Mental Health Assessment to people who are presenting in an acute Mental Health Crisis, and Home Treatment to those people requiring a period of short-term intensive intervention. Under the direction of the senior health and social care staff in the team and/or the shift coordinator you will assist in delivering interventions with an allocated group of service users with mental health needs. The interventions will be focussed on assisting the person to identify and achieve their agreed goals of Recovery. These will include interventions related to; meaningful occupation, accessing employment, assessing and promoting physical health, social inclusion, activities of daily living and mental health promotion. You will also be involved in supporting the service user to complete appropriate outcome measure tools and participate in reviews of the service users care and effectiveness of the interventions provided. Please note this role is shift patterns covering 24/7, you will be required to work night shifts.
