The Discharge hub administrator role will involve Collating and uploading data figures to produce reports in line with stakeholder expectations and report discrepancies to Management. Manage filing systems, and administration tasks, keep accurate records and ensure confidentiality and sensitive information is maintained. Report any data breaches in line with our data protection policy. to produce daily / weekly reports on excel and cleanse and maintain accurate discharge data. Support the team with other administrative tasks as required Assist in dealing with enquiries by telephone or email. Actively and constructively contribute to team meetings. Undertake other duties commensurate with the level of this position Comply with Hillingdon Hospital foundation trust’s policies & procedures.

MAIN DUITES

1. Receive all referrals throughout the day and record / update on

spreadsheet / database.

2. Working in the Departure lounge, booking transport, Completing follow up

call post discharge.

3. Produce the daily discharge list with any confirmed care packages

4. Email all documents to relevant departments within Hillingdon and outside

boroughs and send confirmation e- mail to LB Hillingdon

5. Log referrals from Cerner requesting POC’s and case notes on

the system

6. File out of borough referrals and keep for our records until patient is

discharged.

7. Liaise with wards, Social Services, Private Care Agencies and Out of

Borough Services re Packages of Care (POC)

8. Receive phone calls from various departments from within and outside the

hospital, deal with queries when possible and pass on messages to

Discharge Hub

9. Produce the Long Length of Stay report, inputting into criteria to reside spread sheets and pulling the data as required

10.Type documents when required by the Discharge Hub and on completion

e-mail documents to relevant, integrated care boards (ICB) and

allocated Social Worker via NHS Secure Mail

11.Order supplies as necessary under supervision of line manager and

maintain general office duties on a daily basis
12.Minutes taking at discharge meeting



The Hillingdon Hospitals NHS Foundation Trust is the only acute Hospital in the London Borough of Hillingdon and offers a wide range of services including accident and emergency, inpatient care, day surgery, outpatient clinics and maternity services. The Trust’s services at Mount Vernon Hospital include routine day surgery, delivered at a modern treatment centre, a minor injuries unit and outpatient clinics.
