The Cornwall Home Enteral Feeding Dietetic Team are offering an exciting opportunity for the right person to join us as a Band 4 Dietetic Assistant Practitioner.

This post would be suitable for someone who has a relevant NVQ minimum level 3 or equivalent qualification in health / nutrition / social care, with
experience in a care setting and an interest in nutrition support.

Excellent communication skills and a good level of IT knowledge are essential in supporting the Home Enteral Feeding Dietitians to provide a streamlined but fluid service to our enterally fed community patients.

If you are keen to promote and maximise independence & wellbeing in community patients, have a keen interest in nutrition and nutritional support as well as being organised & efficient then this may be the position you are looking for! Full training and support will be provided by Registered and Gastrostomy Specialist Dietitians to help you achieve the job satisfaction you desire.

Please note that the post holder is currently required to mostly work remotely and necessary equipment will be provided to the successful candidate.

The successful candidate will, with appropriate training and under supervision of the clinical practitioner, be responsible for gathering information to assist the assessment, monitoring and dietetic treatment of enterally fed patients; ensuring care plans are appropriate and are holistically suitable for each individual. This will occur via telephone, face to face or via the use of video contact and may include basic tube and stoma care, where appropriate.

You will be expected to liaise with all the relevant parties (including but not limited to fellow dietitians, the wider MDT and contracted feed
company) to ensure that patients or their carers have the right training & receive suitable equipment to fulfil their individual treatment plans.

Managing incoming referrals and queries to help prioritise dietetic workload is integral to the post. You will work closely with the dietitians,
gastrostomy specialist dietitian, contracted feed company and feed company nurses to ensure a smooth, effective, efficient and streamlined service is provided for our patients, to the best of our capabilities.

Maintaining the HEF service database is central to this role, supporting workload management and enabling completion of annual audit against peninsula standards, of which you will play a key part.
