Position:Complex Care Manager

We are seeking a Clinical Care Managerto lead and oversee the coordination of clientcare services. The ideal candidate will be responsible for managing a multidisciplinary team, ensuring the delivery of high-quality care, and acting as a key liaison between clients, families, and multiagency agencies. You will play a crucial role in improving clientsservices and outcomes, enhancing the clientexperience, and optimizing care delivery.

Responsibilities:

Adopt a person centred approach in all that you do, continually looking to improve how we administer and deliver care in our communities.

Lead and supervise a team of nurses, care coordinators, and other clinical staff to ensure efficient and high-quality care delivery.

Develop, implement, and monitor individualised care plans for clients, ensuring they meet clinical, emotional, and social needs.

Collaborate with healthcare providers, social workers, and family members to coordinate care and ensure a holistic approach to clientmanagement.

Monitorprogress and make necessary adjustments to care plans based on clinical outcomes andfeedback.

Act as a primary point of contact for clientsand their families, providing support, education, and advocacy as needed.

Ensure compliance with all regulatory requirements and organisational policies, maintaining a focus on safety and quality.

Track and report on key performance indicators (KPIs) related to clientcare, team performance, and operational efficiency.

Provide leadership and mentorship to clinical staff, fostering a culture of continuous learning and improvement.

Develop and maintain effective working relationships, particularly with our Business Development teams and Local Authorities so we can acquire new contracts and support packages.

Plan and attend regular meetings with all stakeholders to keep abreast of views, developments, opportunities and expectations.

What we're looking for:

Experienced Registered Adult Nurse

-

Previous experience working in a regulated care environment is a must, ideally in a domiciliary environment/home care.

Excellent communication skills

-

A natural leader, someone comfortable motivating and engaging a team.

Comfortable working within a multi-disciplinary environment.

-

Experience of working with Service Users who have additional support or complex needs.

A passion for delivering excellent care to a high standard.
