Care Coordinator | Your main place of work is Wolverhampton but will include travel twice per month to Cheshire | 40 per week | £25,000-£30,000 per year based on experience

Our client is a Health and Social Care provider, providing dorm care services. The team are highly dedicated to providing and ensuring the highest quality care and service.

If you have proven care experience the employer wants to hear from you! Dorm care experience is an added advantage.

In this role, you will ensure that the care service is delivered to service users at the times and for the duration specified in the relevant individual agreement(s) and contract standards.

Are you the right person for the job?


• 3 years Coordinator experience essential
• Driving Licence and own Car essential
• At least a Level 3 in Health and Social Care
• Ability to draft care plans, assessments
• Good command of English is essential
• You will have knowledge of rotas and dealing with staff members
• Excellent interpersonal skills and the ability to interact well with different kinds of people
• Excellent verbal and written communication skills and the ability to prepare comprehensive reports
• Passion for helping people and providing personalised care
• Strong leadership and management skills and the ability to motivate others
• In-depth understanding of the organisation’s policies and medical regulations
• Ability to maintain a high level of confidentiality and respect for patients
• Proficiency with computers and Microsoft office tools such as Word, PowerPoint and Excel
• Strong analytical skills and the ability to handle multiple tasks at a time
• Recruiting, training and supervising staff and communicating the needs of patients to Medical Staff
• Reliably commute or be willing to relocate to Cheshire


What will your role as a Care Coordinator look like?


• Liaising with local community organisations and maintaining good relationships with stakeholders
• Ensuring that all activities such as personal care and medication administration meet the required standards
