North East Derbyshire (North) Adult Social Work Team are seeking to recruit experienced Social Workers to join our friendly, community-based team of Social Care Practitioners and Social Workers. The team are highly supportive and operates on a largely agile, flexible basis across North East Derbyshire, utilising link worker and champion roles to carry out Care Act (2014) telephone and face-to-face assessments and reviews.
The right applicant will need to evidence high levels of autonomy and accountability with timely recording of interventions to progress overall case management. Applicants must ensure they can demonstrate the skills to build positive, professional links with others to work closely with our partner agencies, such as Health, Police, Housing and PVi sectors. The ability to identify signposting opportunities is necessary to support carers, maximise independence and individual wellbeing with a focus on promoting person-centred, strength-based outcomes.
At times the role requires Duty cover, it is necessary to be able to respond to changing situations, to effectively triage and assess incoming work to support Adults and their carers - who may require a more urgent response. It is vital to be able to evidence experience and skills to manage a complex caseload whilst meeting your statutory duties in a safe, accountable and cost-effective manner to meet Care Act (2014) eligible needs alongside local authority statutory duties of care.
It is an advantage to have experience of taking the lead on Section 42 Safeguarding, chairing multi agency meetings, such as Multi-Agency Adult Risk Management (MARM) and completing Mental Capacity Act (2005) assessments, including Best Interest meetings, COP-DOLs and court work – though support, training and experience will be available for the right candidate if you wish to develop your skills in these areas.
