The care and support we provide is highly valued by the people we care for and their loved ones, but at present we are only reaching around 10% of dying people at the end of life.Right now, one in four people in the UK with a terminal illness, do not get the care or support they deserve at the end of their lives. We are recruiting for a Healthcare Assistant to join our community-based team in Bristol. Hours:Full-time night shifts, as per rota (weekdays and weekends) Location:within a 25-mile radius to your address (Bristol area).This role is based within a congestion zone, but charge will be reimbursed. In this role, you will work alongside a multidisciplinary team to provide assessment, care planning, review and evaluation of holistic care management plans individual to each patient. As this is a community-based role, you will be responsible for planning your own work, responding to changes in patient conditions, prioritising care needs (including hygiene, nutrition and hydration, comfort, monitoring vitals, and more) as detailed in the personalised care plan and ensuring you escalate appropriately to senior staff within the multidisciplinary team in a timely manner.
