KEY DUTIES To assess, plan, implement and evaluate care for cardiac patients with a variety of cardiac conditions. To work autonomously and manage own specialist defined patient caseload. To deliver cardiac rehabilitation programmes phase 11 and 111. To be involved in developing specialised, evidence-based service delivery which is linked to both local and national targets. Liaise with all members of the MDT involved in patient care including GP, Cardiologists, etc. Provide clinical supervision and mentorship. Complete a holistic assessment of risk factors and offer appropriate lifestyle advice includes exercise and educational component as well as psychological support and lifestyle modifications. Refer patient to appropriate services for ongoing care. Deliver nurse led clinics and home visits post cardiac event clinics where patients recovery symptoms and treatments are assessed. Advise/undertake drug titration and post MI clinics Collection and input of NACR (National Audit of Cardiac Rehabilitation. Work in collaboration in the development and delivery of coronary heart disease training for the Primary Healthcare Team CLINICAL & PROFESSIONAL RESPONSIBILITIES Maintain Professional Registration Adhere to the NMC Code of Practice and The Scope of Professional Practice Ensure that nursing care is appropriate, provided and is consistent with clinical guidelines In accordance with the Health and Social Care Act 2008, the post holder will actively participate in the prevention and control of infection within the capacity of their role. The Act requires the post holder to attend infection prevention and control training on induction and at regular updates and to take responsibility for the practical application of the training in the course of their work. Infection prevention and control must be included in any personal development plan and/or appraisal To report and record any untoward incidents To assist in the promotion of effective communication with all staff, patients and relatives and other departments as required ADMINISTRATIVE RESPONSIBILITIES Offer clinical supervision to other colleagues TEACHING & TRAINING RESPONSIBILITIES Assist in the development of an education program for key areas Be involved in staff education and development Maintain and develop personal knowledge and skills in the field of cardiology nursing Develop audit skills and undertake training as required RESEARCH & AUDIT Assist with relevant audit projects. Work in collaboration with the audit department to ensure correct data entry Assist with the development of appropriate audit tools and database Assist in collecting all the required information Assist with measures to improve performance as required To comply with the Data Protection Policy of the Trust Involvement in research projects as appropriate
