In this role you will be responsible for the provision of occupational therapy assessment, treatment and evaluation. You will provide visible and professional support to ensure the delivery of high quality patient care and rehabilitation aimed at improving or maintaining independence. Working with a caseload of patients with varying difficulties including complete health and social care needs; responding effectively to the changing needs of the patient caseload, managing the sometimes conflicting priorities of prevention of admission and discharge. Youll assess and order equipment as identified by patient need and allocate work to OT assistant or care staff as necessary. Establishing communication networks with other team members, patients and other health and social care agencies, attending joint visits with other professionals as required. As an occupational therapist working with Practice Plus Group you will: Provide Outstanding evidence based care, listening and acting on patient feedback so that care is personalised and informed by what matters to them. You will provide treatment for patients with complex, severe and enduring needs. Ensuring written and electronic records and activity data are maintained in accordance with Practice Plus Group policies Deliver Non-judgemental care, building therapeutic relationships and promoting professional boundaries. You will implement occupational therapy interventions, provide baseline assessment of patients level of functioning and identify care planning issues to devise realistic and appropriate treatments plans. Arranging/attending case conferences as appropriate to patient need.
