We currently have vacancies for experienced social workers to support individuals with a Learning Disability and associated mental health diagnosis for adults transitioning young people into adult services. The team provides consistent and intensive case management during initial stages of a service being set up and where the professional view is that support can be reduced over a period of time e.g. over the first years of care. It provides a level of assurance for the management team (DMT) in terms of having oversight across packages of support which may involve a deprivation of liberty and where there is a requirement to regularly review the restrictions which are in place to keep the person safe.

You will be based in the High Needs team, at the guildhall in Hull, working collaboratively with mental health and learning disability heath colleagues, to plan, prepare and deliver bespoke services for people with complex needs.
You will ensure through ongoing case management that these services continue to meet needs and are best value, proportionate and least restrictive for their assessed need.
Through case management, you will be the specialist social worker to inform multi agencies, such as Mappa, mental health tribunals, and coordinate the mental health provider involvement to ensure there is not duplication of effort.

Ability to establish professional, effective working relationships with a range of partners/colleagues, in children’s services through transitions, adults, families and their carers
You will hold a professional social work qualification registered to Social Work England and will have experience of assessment and planning, initiating, monitoring, developing and providing outcomes-driven services in adult social care.
