It is an exciting time for complex discharge with many new pathways and initiatives being introduced with partnerships with external agencies being forged by the complex team.
You will provide clinical expertise to support safe, effective and timely discharge working directly with internal and external multidisciplinary teams as required.
The role will coordinate the NHS CHC process including carrying out nursing assessments ensuring that the Care Domains within the national framework for NHS continuing healthcare are applied against each continuing healthcare assessment and a detailed rationale given for each decision made. You will also recommend based on clinical evidence if patients have a 'primary health need' and therefore are eligible for NHS continuing healthcare.

• You will be responsible for supporting and facilitating the safe discharge of patients from Ipswich Hospital and the community hospitals in Ipswich and East Suffolk, and the effective delivery of Discharge to Assess.
• You will be responsible for supporting all aspects associated with the facilitation of discharging patients from Ipswich Hospital and the community hospitals, working with the relevant clinical specialties of ESNEFT to agreed quality standards and within the resources and income base available, to support the Trust’s development as a clinically led organisation.
• To work collaboratively with other clinical services within the hospital, social care, voluntary services, care sector, community services, and across the Alliance partners.
• Will provide that trusted assessor role to our care home providers.
• You will be responsible for interpretation of national policy and guidance relating to the safe discharge of patients, ensuring the development and implementation of local policy and procedures to deliver a quality integrated discharge service.
• To provide specialist advice with respect to NHS EOL FastTrack Continuing Healthcare, to patients, health and social care professionals and other agencies.
• To provide support and challenge to ward pressures to ensure high quality individualised discharge planning for patients and relatives/carers and to contribute to the continuing improvement and development of discharge processes and care pathways.
