Job Purpose To contribute to a high-quality service by carrying out assessment, goal setting, implementation and evaluation of treatment programmes, within patients own homes, of adult clients who may have complex and/or chronic presentations including musculoskeletal, sensory impairment, medical and mental health needs, with support and supervision from Senior Physiotherapists. To support clients to improve or maintain their level of function and quality of life, and in particular to reduce the risk of falls, in partnership with the multi-disciplinary team (MDT). This will include liaising with and referring on to other health services, voluntary services and public sector agencies, and planning safe and effective discharge from the service. To participate actively in the services training strategy, including in-service training, organisation-specific and multi-agency training. Dimensions Responsible for Physiotherapy assessment and treatment for a defined clinical caseload appropriate to the service area, working in conjunction with and under supervision of a Senior Physiotherapist and other senior clinical professionals. To participate in rehabilitation treatment programmes with the MDT, including leading group rehabilitation classes as appropriate. To contribute to the supervision and training of Assistant Practitioners and Rehabilitation Support Workers, and to support senior colleagues with delivering student placements Able to fulfil physical demands of role, including manual handling applications. Will be required to travel city-wide for patient visits, meetings and training. The role has no specific budgetary responsibility, however there will be a requirement to demonstrate cost-consciousness within the role. Key Responsibilities Clinical and professional Responsible for physiotherapy assessment and treatment for a defined clinical caseload, working in conjunction with and under supervision of a senior physiotherapist and other senior clinical professionals. To be professionally and legally responsible and accountable for all aspects of your own work including the management of patients in your care. To ensure a high standard of clinical care for the patients under your management, and support other team members to do likewise, implementing appropriate teaching, supervision and delegation skills where appropriate. To work holistically across role boundaries to Band 4 level to assess for, provide, demonstrate and fit adaptive equipment using problem solving, and analytical skills. To organise own clinical caseload effectively. Working within a legal framework, gain informed consent to treatment and have the ability to identify, through assessment, patients who lack the capacity to consent. To demonstrate a sound understanding of Clinical Governance and Risk Management and apply to a work situation. To conduct and report on clinical risk assessments as and when appropriate. To undertake the comprehensive assessment of patients, including those with a complex presentation with the support of senior physiotherapists. Formulate individualised treatment plans with rehabilitation goals using clinical reasoning in consultation with senior therapists as required. This should be based on sound theoretical knowledge, utilising a wide range of treatment skills, and options to formulate a specialised programme of care. To advocate on behalf of patients and carers, where appropriate, overcoming barriers to communication such as cultural or language difficulties, physical or mental health difficulties, using skills of empathy to facilitate or empower the patient. To be involved in the development of good practice for physiotherapists within Nottingham CityCare Partnership CIC and Social care under the guidance of senior staff. To complete patient records in line with organisation, regulatory body and legal requirements. To demonstrate a commitment to evidence-based practice and the use of outcome measures and make recommendations for change. To have a good understanding of the research process, and be willing to participate in local research and audit when appropriate. To be responsible for own Continuing Professional Development, keeping abreast of new recommendations, research and developments, incorporating them into your work as necessary To maintain a high level of professional standards and work to the Professional Code of Conduct as set by the Health and Care Professions Council. Management and Leadership To undertake day to day clinical prioritisation and work planning, supported by senior physiotherapists. To establish positive and mutually supportive relationships with all team members and contribute constructively to multi-professional team working To promote the roles and skills of Physiotherapy staff to other professionals. To demonstrate a can do attitude and support others to do the same. To demonstrate an understanding and application of the CityCare values and 6 Cs of compassionate care within daily practice, and support others to do the same. Policy To be actively involved in Clinical Governance, implement policies and proposing practice changes. To comply with organisations Lone Working Policy and local team guidelines at all times when working alone. Planning and Organisation To be responsible for own time and caseload management. To liaise effectively with other professionals and agencies to enhance rehabilitation within the boundaries of Nottingham CityCare Partnership CIC and Social Care. To use information technology on a daily basis to record information and statistics, to aid communication, and produce reports. To ensure the Health and Safety of staff and patients at all times in accordance with Health and Safety legislation, and CityCare policy and procedures. To undertake any other duties which may be reasonably required. Training To comply with and complete statutory and mandatory training in line with Nottingham CityCare Partnership policy. To attend training events as appropriate for your continuing professional development. To actively participate in your own supervision, annual performance review and personal development plan
