Salary: Grade J. Scale 40 - £49,764 to Scale 43 - £52,805 with a professional bar at SCP 41 £50,788**

The Role

We are looking for a dynamic and motivated Team Manager to join us within the Integrated Urgent Care Team (IUCT). The team provides short term interventions to maximise independence and responds to urgent/crises situations to enable people to continue living in their own home for as long as possible. We facilitate timely discharges from hospitals and the intermediate care facility.

Our aim is to support people to live well at home and we do this by working in our integrated team which include Social Workers/Assessors, Physiotherapists, Occupational Therapists, Nurses, Technical Care Instructors, Assistant Practitioners and Community Care Officers. About You

As a key member of this busy and collaborative team you will be responsible for managing and overseeing the day to day operations of the service as well as line managing and providing support, advice and guidance to social care practitioners within the team.

The core objectives of your role will be to assist the Service Unit Manager and Assistant Team Managers in the organisation, management and supervision of Adult Social Care services. Contribute to overall strategic planning and management of the service and to the development of high quality integrated services.

As a member of a very supportive management team you will assist in leading the team ensuring that the functions of the Unit are carried out effectively and in accordance with legislation, regulations, guidance, standards and local procedures and priorities. You will be expected to drive forward the vision of continuous improvements within the service in light of service user views, performance data and stakeholder feedback and enhance collaborative working across all partners inside and outside of the organisation to achieve better outcomes for people. You must be able to lead and provide effective communication for both users of the service as well as staff teams and hold a desire to promote positive working approaches in all situations, and the ability to remain calm and reassuring in challenging situations.

You will receive supervision, peer support and training to ensure your continuous professional and personal development. We want you to experience strong and supportive leadership where you are engaged and be able to actively participate in our transformation journey.

It is essential that you hold a current valid driving licence and have access to a vehicle for work use. The post also requires an enhanced DBS disclosure.
