To work as part of the multi-disciplinary urgent response service to support unwell patients in domiciliary and care homes. This role will perform urgent venous blood sample collection to service users over the age of 18 years and a limited range of basic nursing tasks. The service operates from 0800-2000hrs 7 days a week. To work as a member of a multidisciplinary team providing clinical care for patients within research and evidence-based guidelines. The team comprises of advanced clinical practitioners, paramedics, phlebotomists, therapists, care managers and social care providers. In the role as Clinical Support Worker, working without direct supervision you will be responsible for delivering clinical care for patients in their own home. With Immediate access to a qualified staff member for advice and direction on care as required. You will be responsible for your prioritising your own caseload of work and be responsible for tasks delegated from a registered nurse. Assist in preparing patient notes and relevant charts/documentation on admission to the service, prior to the first home/clinic visit and as required. Assist the registered nurse in the prevention of pressure damage and care of patients pressure areas, recording and maintaining relevant documentation and escalation as required. When necessary initiate and assist with any emergency procedures including basic cardio pulmonary resuscitation. Perform and record (in real time) patients vital signs as directed, reporting findings to the registered nurse on duty Temperature, Blood Pressure, Respiration, Oxygen Saturation and Heart Rate. Reports patients condition to qualified staff and escalate appropriately early warning triggers in line with service standard operating procedures. Perform venepuncture as directed by the registered practitioner, ensuring relevant documentation is completed and PPE policy followed. Perform additional clinical skills as directed by the registered practitioner where trained and competent to do so within the scope of the role and speciality. At the request of the registered practitioner, perform Blood Glucose levels finger pricks, utilising blood glucose monitoring equipment recording results within the patients record. Where abnormal results are identified, escalate to the registered practitioner on duty. Collect Midstream and Catheter specimens as per organisational policy. Coordination of Virtual Wards including the monitoring of a patient clinical dashboard Virtual Ward equipment implementation and monitoring.
