Hourly rate of pay: £24.24 - £27.07


Candidates will be assessed on the basis of appropriate relevant experience and qualifications and salary placed within the advertised scale.


Days and Hours of Work:
Monday - Thursday, 09:00-17:00
Friday, 09:00-16:00
Days of working are negotiable, starting and finishing times are flexible, and flexi time is applicable.

We are looking for a highly motivated and enthusiastic Senior Practitioner interested in the challenges of working in our Hospital Discharge Team and who can bring their knowledge, skills and experience. The team is based within East Lothian Community Hospital and works closely with NHS colleagues.


The role of the team is to assess adults in hospital who may require to move into a care home or support on discharge home and also provides a review and adult protection service to adults living in care homes. The Senior Practitioner acts as a line manager within the team, overseeing the work of Social Workers and CCWs and providing supervision and support. Ideally , we are looking for someone with experience of working in hospital and community settings with a sound knowledge of self-diected support and resources available to support people in their own homes

Job Requirements

Education/Qualifications
Essential: Approved qualification in Social Work.
Registration with Scottish Social Services Council (SSSC).
Those with international Social Work qualifications should not apply unless they are fully registered with the SSSC.
Desirable: Post qualifying award


Driving Licence
Full current driving licence and access to a vehicle*
